Rob Reiner's Misery (1990) earned Kathy Bates the Academy Award for Best Actress—the first horror performance to win the award—for her terrifying portrayal of Annie Wilkes, the "number one fan" who holds author Paul Sheldon captive. The infamous hobbling scene, where Annie breaks Paul's ankles with a sledgehammer, was actually toned down from the novel where she amputates his foot with an axe, yet remains one of cinema's most wince-inducing moments. James Caan delivers an equally strong performance as the immobilized author, forced to write a new novel to satisfy his captor's obsessions. The film works as both horror and a meditation on the relationship between artists and their audiences, with Annie representing fan entitlement taken to murderous extremes.
